This Guide to Transition for Mapledown School Leavers tells you what transition is and what you can expect from the transition process.
The guide will show you who is involved at which points and tells you about the various planning stages to ensure that when a student leaves Mapledown all of their social and health needs are met by adult services.
Any transition can be an anxious time for everyone involved and the aim of this is to make the process as transparent as possible. The whole process is person centred and involves families and carers every step of the way.
